APPOINTMENT FEES
Initial Consultation (60-80 minutes): $600-900
Follow Up Sessions (25-50 minutes): $313-500
INSURANCE INFORMATION
Although insurance is not accepted, if you have out-of-network mental health benefits, you may request a "superbill" to submit to insurance for reimbursement.
COMMERCIAL INSURANCE
KAISER
Please specify if you have Kaiser. A regional agreement is in place for clients in Northern California. With prior authorization from your doctor, Kaiser may be billed directly for services. If Kaiser is secondary and you have another primary insurance, you must indicate your primary insurance. The primary insurance must be billed first. If your insurance does not cover the visit (including due to an unmet deductible), you are responsible for the full cost of services. Any balance not paid by insurance will be charged to your credit card on file after insurance processing.
MEDICARE
Please specify if you have Medicare. There is limited appointment availability for Medicare clients and a separate Medicare-approved fee schedule applies.
