TREATMENT PHILOSOPHY
Many of the conditions I treat may over time result in loss of independence and significant life limitation. My style is collaborative. I believe in a therapeutic relationship which fosters the development of autonomy and sense of mastery, which are key to recovery. Techniques used include cognitive behavioral therapy (CBT), dialectic behavioral therapy (DBT), acceptance and commitment therapy (ACT), psychodynamic methods, relaxation skill building, mindfulness and hypnotherapy. I may provide medication recommendations to the primary treating team.
FOLLOW-UP VISITS
Depending on circumstances, I may either communicate a treatment plan to a client's local health care team or recommend periodic follow-up visits together. These typically last 25-50 minutes. During visits, clients are guided in areas including self-observation, mindfulness, skills development and effective communication. Hypnotherapy may also be an option, with the opportunity to learn self-hypnosis if interested.
INITIAL CONSULTATION
I accept client self-referrals plus those from neurologists, psychiatrists and others. Consultation appointments are by way of HIPAA-compliant video, also known as telemedicine. A typical consultation lasts an average of 60-80 minutes, during which a collaborative treatment plan is developed. Recommendations are tailored to each individual and provided in written format after the visit.

HEALTHCARE TEAM CONSULTATION
Collaborating with professionals is an important part of every client interaction. With client permission, I provide a written report to referring physicians. Professional education resources and coaching are also available upon request.
FEE SCHEDULE
Initial Consultation (60-80 minutes): $600-900
Client Follow Up Visit (25-50 minutes): $313-500
No Show or Same-Day Cancellation: $50

Although insurance is not accepted, if a client has out-of-network mental health benefits, they may request a "superbill" to submit to insurance for reimbursement.
Please specify if you have Medicare. There is limited appointment availability for Medicare clients and a separate Medicare-approved fee schedule applies.​

HOW DO I GET STARTED?
If you have a diagnosis of Functional Neurologic Disorder (FND), please click here for the referral form for your neurologist to complete. Please ask them to send the form plus select records (only those listed on the referral form) by fax 408.531.6751 or secure email consult@lockmanmd.com. Once these are reviewed, I will reach out to schedule.
